GROUP CLASS POLICY AGREEMENT

Payment and Refund Policy

· Full payment for any training class is due by the first session.

· If cancellation occurs less than 10 days prior to the class start date, a $30.00 administration fee will be deducted from paid fees.

· Once class begins, the class fee is non-refundable. Individual considerations may be made for true emergencies or sickness. Reschedule to another class will be the first option.

· Should the trainer decide to refund the fee, a $30.00 administration fee will be deducted from its total.

· Payments made with an insufficient check will incur a $20.00 fee, and will not be given a refund of any kind.

· There is no “money back guarantee”. Refer to training results section below.

Attendance Policy

· Attendance is vital for successful learning of training techniques.

· Missing a class is the client’s decision and the trainer is not required to refund monies or make up the missed time in any way. Make-up sessions will be completed upon the trainer’s discretion.

Training Results

· The client assumes full responsibility for the conduct of their dog at all times. Should an altercation or injury occur between/to any person or animal, the trainer, Mad4MyDog Training LLC, and the host facility cannot be held liable.

· Mad4MyDog Training LLC will provide the knowledge, technique, and skills required for successful dog training. The client is required to use that information given by Mad4MyDog Training to train the dog on their own. Mad4MyDog Training LLC cannot be held responsible for inadequate utilization of the learned dog training principles which may result in undesirable results from the dog being trained. This holds true for the dog’s behavior now and in the future.

PRIVATE TRAINING SESSION AGREEMENT

Payment and Refund Policy

· Full payment for Private Session packages is due by the first session.

· Refunds are entirely up to the trainer’s discretion. Individual considerations may be made by the trainer for true emergencies or sickness.

· Should the trainer decide to refund the fee, a $35.00 administration fee will be deducted from its total.

· Payments made with an insufficient check will incur a $20.00 fee, and will not be given a refund of any kind.

Session Rescheduling Policy

· Rescheduling must occur at least 24 hours ahead of the session’s scheduled start time.

· All private sessions must be completed within 4 months of first session date or will result in forfeit.

· 2 Session Package - 1 reschedule is permitted. Any beyond that may result in a session forfeit.

· 4 Session Package – 2 reschedules are permitted. Any beyond that may result in a session forfeit.

Training Results
