Enrollment Procedure
Please review the POLICIES before enrolling for class.
STEP 1: To enroll in ANY CLASS/SEMINAR, complete and send the electronic enrollment form below. FOR CLASS PACKAGES: Enter the class date as TBD (to be determined).
STEP 2: WAIT. Your electronic form will be reviewed within 2-3 business days and accepted if there is space available. You will receive an email reply with your acceptance into class and further instructions.
STEP 3: FOR BASIC FOUNDATION CLASS & PACKAGES: Print the Client Enrollment Form & Class Policy Agreement, complete them, and mail along with your check payment to: Mad4MyDog Training LLC, 5857 W Delap Rd, Ellettsville, IN, 47429. Your space in class is NOT reserved until payment is received.
